Registration

registration_formThe Reception staff will explain the process of how to register with the practice and will ask you to complete a set of Registration Forms. Once these are completed and returned, all new patients are then asked to attend a New Patient Appointment.

For this appointment it is important that you bring with you a urine sample clearly labeled with your name & date of birth (suitable containers are available from Reception) and all your current repeat medications (or a current re-order form).

It is now current practice to be registered with a Practice, not an individual GP.

Geographical Boundary

Each medical practice has a geographical boundary. Patients living within the boundary are entitled to register with the Insch Medical Practice. If you move house and your new home is located outside this boundary, we will write to you and ask you to register with another practice.

Our geographical boundary is displayed on a map, in the Health Centre. A receptionist can give you access to this, if you wish.
